R&B Labs, LLC - Service Agreement
1. Services Provided
1a. R&B Labs does not collect any specimens without a physician's lab order or request from a laboratory.
1b. R&B Labs agrees to provide mobile phlebotomy services to the patient as specified in the order and as mutually agreed upon by both parties. These services include but are not limited to blood draws and specimen collection.
1c. R&B Labs is responsible for drawing and delivering the collected specimen to the laboratory.
1d. R&B Labs does not perform any testing on the specimen.
1e. R&B Labs is not responsible for any errors that occur at the laboratory.
2. Appointments
2a. Appointments for mobile phlebotomy services will be scheduled in advance and agreed upon by R&B Labs and the patient.
2b. The patient shall be available at the scheduled time and location.
2c. In the event of any changes or cancellations, the patient agrees to notify R&B Labs at least 24 hours in advance.
3. Payment and Billing
3a. The patient agrees to pay R&B Labs the agreed-upon fee for the phlebotomy services rendered. Payment is due at the time of service unless other payment arrangements have been made in advance.
3b. The patient acknowledges that additional fees may apply for services requested outside regular business hours, additional travel distances, or other special requests.
3c. R&B Labs does not bill your insurance for these services. Any billing questions or concerns should be directed to your insurance company or the laboratory billing department.
4. Patient Responsibility
4a. Provide accurate and complete medical information and any relevant medical history to ensure safe and effective phlebotomy services.
4b. Follow any pre-collection instructions provided by their physician or by R&B Labs, including fasting requirements.
4c. Cooperate with the phlebotomist during the blood draw or specimen collection, ensuring a safe and efficient process.
5. Confidentiality
Both the company and the patient agree to maintain the confidentiality of all medical information and test results obtained during the provision of phlebotomy services.
6. Consent
The patient gives informed consent for the collection of blood or specimens, understanding the purpose and potential risks involved.
7. Liability
7a. R&B Labs shall exercise reasonable care and skill in providing mobile phlebotomy services. However, R&B Labs shall not be liable for any complications or issues arising from the service, unless they result from R&B Labs’ gross negligence or willful misconduct.
