What You'll Need to Schedule 

  • Submit an appointment request form

  • Physician's lab order 

  • Insurance card for laboratory billing 

Good to know

  • Appointment times are scheduled within a 1 hour arrival window

  • Your appointment is not confirmed until all required documents are received

We do NOT accept insurance for mobile draws.
